It is important for you to know how much your insurance company will cover of my fee. I will do my best to work with you in setting a fee that works for your financial situation. Please call the number on the back of your Health Insurance Card.

I am an out-of-network provider for PPO plans. I am NOT a provider for HMO plans except those listed on my website, i.e. UBH.

How to Verify Your Insurance Mental Health Benefits:

Ask for Out Patient Mental Health Benefits
• Ask if they cover Licensed Marriage Family Therapists

There are two different kinds of reimbursement depending on your diagnosis.  As I cannot determine your diagnosis without meeting you, please verify both:

Scenario A:  Parity Diagnosis (major depression, panic disorder, etc.)
• What percentage of my fee do they cover?
• What is your copay per session?
• How many sessions a year do they allow?
• Have you satisfied your deductible (if no, how much is your deductible)?

Scenario B:  Non-Parity Diagnosis
• What percentage of my fee do they cover?
• What is your copay per session?
• How many sessions a year do they allow?
• Have you satisfied your deductible (if no, how much is your deductible)?

IMPORTANT:  Ask and obtain a pre-authorization for your sessions if needed.
