It is important for you to know how much your insurance company will cover of my fee. I will do my best to work with you in setting a fee that works for your financial situation. So we can do that please call the number on your Health Insurance Card.

I am an out-of-network provider for PPO plans. I am NOT a provider for HMO plans except those listed on my website, i.e. PacifiCare.

How to Verify Your Insurance Mental Health Benefits:

Ask for Out Patient Mental Health Benefits
• Ask if they cover Licensed Marriage Family Therapists

Also ask what are the benefits for a Parity Diagnosis
• What percentage of my fee do they cover?
• How many sessions a year do they allow?
• Have you satisfied your deductible (if no, ask how much your deductible is)?

Also ask what are the benefits for a Non-Parity Diagnosis
• What percentage of my fee do they cover?
• How many sessions a year do they allow?
• Have you satisfied your deductible (if no ask how much your deductible is)?

Ask for the billing address so I can bill for you (you will pay the co-pay only).
